    About the Program

    The 2025 Safety Recognition Program focused on companies and facilities in four key areas, Process Safety, Emergency Planning & Response, Stakeholder Engagement & Communications, and Safety Leading Indicators. It recognizes those who have implemented effective systems and processes focused on safety in all four areas.

    Safety and Risk Management

    ACCI Specialty Materials LLC

    ACCI Specialty Materials LLC

    ACCI Specialty Materials is recognized for its proactive and collaborative approach to process safety, emphasizing hands-on learning and continuous improvement.

    Through enhanced inspection protocols and proactive lifecycle management, ACCI Specialty Materials has eliminated hose-related incidents and elevated overall operational safety. The introduction of hose numbering, routine inspections, and clearly defined service-life limits has substantially reduced risk across facility operations. Employee engagement is further driven by dynamic training programs and performance incentives, while close collaboration with local fire departments ensures first responders are fully prepared for chemical emergency response.

    Ashland

    Kenedy, TX

    The Ashland Kenedy facility is recognized for strengthening workforce competency and hazard awareness through targeted training, proactive analysis, and continuous engagement in safety improvement.

    After identifying a training gap in barricaded work areas, the Kenedy facility introduced new procedures, updated orientation programs, and reinforced awareness through targeted instruction. The structured operator certification program now incorporates hands-on evaluations, written exams, and management reviews to ensure both readiness and accountability. These initiatives—supported by quarterly behavior-based safety activities and more than 250 “good catches” reported annually—have strengthened hazard recognition and increased employee ownership of safety performance. Leadership leverages culture survey results, Gemba walks, and leading indicator data to drive tangible improvements, including enhanced forklift traffic controls that have successfully eliminated related near misses.

    Lima, OH

    Ashland’s Lima facility is commended for strengthening operational safety through procedural improvements, ergonomic design, and consistent process safety management.

    Following a minor railcar derailment, the Lima facility strengthened procedures to limit car movement during switching and carried out targeted maintenance to address equipment vulnerabilities. Regular process safety meetings and task observations now help identify issues early, while ergonomic enhancements—such as relocating sample points and installing new safety showers—have reduced physical strain and improved worker comfort. These proactive measures, reinforced by disciplined safety leadership and active employee engagement, have supported an outstanding milestone: 15 years without a recordable injury and five years without an environmental incident—underscoring Ashland’s enduring commitment to safety and environmental excellence.

    Hopewell. VA

    Ashland’s Hopewell facility is recognized for advancing safety performance through robust training, data-driven leading indicators, and innovative safety management technology.

    The Hopewell facility leverages a sophisticated Safety Management System (VelocityEHS) to monitor and manage leading indicators across all dimensions of process safety. Real-time dashboards, automated workflows, and AI-guided risk evaluations enable trend identification, strengthen compliance, and ensure rapid response to emerging issues. Employees receive comprehensive training in process safety management, hazard analysis, and contractor oversight, supported by continuous feedback loops that refine programs and drive engagement. Daily and monthly scorecards provide visibility into critical metrics—ranging from near misses and corrective actions to equipment inspections—empowering teams to make proactive safety improvements. This data-driven approach fosters accountability, prevents incidents, and underscores Ashland’s sustained commitment to operational excellence and employee well-being.

    Columbus

    Ashland’s Columbus site is honored for improving equipment safety and operator accountability.

    Following a minor reactor sight glass failure, the Columbus facility proactively replaced all units with armored versions to strengthen containment integrity and prevent recurrence. Employee engagement remains central to safety performance, with 100% participation achieved through recognition programs such as “Safety Leader of the Month” and the “Safety Participation Hall of Fame.” Leading indicators—including near misses, audits, and behavior-based observations—are closely tracked and reviewed during monthly safety calls across all plants. A recent two-day ergonomic RAPID event generated 65 improvement opportunities, underscoring the team’s commitment to continuous improvement.     Rayne Blend Plant

    Baker Hughes’ Rayne Blend Plant is recognized for its data-driven process safety excellence and proactive maintenance culture.

    The Rayne Blend team introduced targeted training on heat exhaustion, with a focus on raising awareness of medications that can affect temperature regulation—an often-overlooked risk factor in high-heat operations. Through participation in the Leading in Human and Organizational Performance (LHOP) program and structured “Walk Through/Talk Through” reviews, employees actively identify potential hazards before they lead to incidents. Recent enhancements, including redesigned tote scales and clearer product labeling, have reduced error potential while improving operational efficiency. All action items, investigations, and near misses are managed in the Intelex system, ensuring transparency and accountability. A strong culture of communication and care empowers every team member to speak up, reinforcing collaboration, safety ownership, and overall well-being.

    Bakersfield Blend Plant

    The Bakersfield Blend facility is recognized for strengthening workplace safety through proactive hazard reporting and continuous improvement in maintenance safety.

    Following near misses involving forklifts, Bakersfield lowered vehicle speeds to 5 mph and strengthened traffic safety through focused training and continuous monitoring. Using the Intelex system, employees report hazards, near misses, and stop-work events in real time, enabling rapid corrective action and ensuring transparency. Additional improvements—such as consolidating unused waste accumulation areas and shifting to bulk waste removal—have reduced handling risks. By prioritizing safety observations and near-miss reporting, the team identified and repaired multiple leaking valves, demonstrating how active employee engagement directly drives safer and more efficient operations.

    Taft Manufacturing Company

    Baker Hughes’ Taft Manufacturing Company is honored for its team-based safety initiatives and open communication practices.

    After detecting a leaking cylinder during waste disposal, the Taft team swiftly contained the issue and worked with contractors to ensure its safe removal. All incidents, near misses, and stop-work reports are captured in real time through the Intelex system, enabling rapid response and accountability. Toolbox meetings provide a forum for employees to review safety data and share observations, driving corrective actions such as repairs to the site’s perimeter fence. Collaboration with county firefighters and environmental health officials—through classroom training and site tours—further strengthens emergency preparedness and transparency. Together, these efforts reflect the facility’s commitment to continuous improvement and to building strong community partnerships in maintaining a safe, responsive operation.

    Bayport, TX Facility

A blended training program—combining online modules with hands-on instruction—reinforces safe practices, while community outreach initiatives such as Adopt-A-Highway cleanups, food drives, and participation in the National Cornbread Festival highlight a lasting commitment to safety and community. The Colonial Chemical team also prioritizes behavioral safety, thorough incident reporting, and preventive maintenance to minimize process risks.     Iofina Chemical

    Covington, KY Facility

    Iofina Chemical is commended for its strong focus on data-driven safety management and a culture of accountability.

    Iofina identified degradation issues in laboratory glassware that posed potential risks and responded with a weekly inspection and weighing program to detect and remove compromised equipment. Since adopting this preventive measure, the site has maintained reliable, incident-free performance in this area. A culture of accountability is reinforced through employee-led safety committees, quarterly all-hands meetings, and routine near-miss reviews that engage every department. Beyond operations, Iofina partners with local emergency responders through facility tours and drills and supports community initiatives such as food and toy drives—demonstrating its commitment to both operational excellence and community care.

    Oakwood Products Inc.

    Estill, SC Facility

    Oakwood Products Inc. is recognized for strengthening laboratory safety and emergency preparedness through procedural improvements, realistic training, and community collaboration.

    Following a near miss involving glove box operations, Oakwood strengthened its safety protocols by requiring dual verification of chamber closure before handling reactive materials. This safeguard is reinforced through scenario-based training, interdisciplinary emergency drills, and peer observation programs that promote precision and accountability in lab practices.     BSC: Specialty Place has redefined safety by embracing a Human and Organizational Performance framework that emphasizes system improvements over individual blame. Through cross-functional Learning Teams, the company investigates issues—such as rupture disk failures—to uncover root causes and implement corrective actions before incidents occur. The adoption of human-performance tools has simplified near-miss reporting and increased employee participation.     CJB Cypress Street

    CJB Industries’ Cypress Street facility is commended for its commitment to continuous learning and proactive safety innovation.

    At Cypress Street, near-miss data and observation findings drive continuous improvement across operations. Through its quality mini-audit program, the facility identified and corrected labeling and storage deficiencies, reaching full compliance within a single quarter.     CJB Cypress Warehouse Plant

    A recent noise monitoring survey at InChem identified sound levels above 85 dB during contractor equipment cleaning. In response, the company introduced new controls and ensured ear protection is readily available for all personnel involved in or near cleaning activities. Ongoing safety training—delivered through weekly toolbox talks, monthly online courses, and annual hands-on sessions—reinforces safe work practices and continuous improvement.     Allen Plant

A comprehensive training program—combining classroom instruction, hands-on practice, and scenario-based learning—keeps employees actively engaged, while multi-agency emergency drills with fire departments and state officials underscore the company’s commitment to readiness and community protection.

    MFG Chemical, LLC

    Brooks Road Plant

    After a near miss involving the safe work permitting process, the Kimberly Park Plant provided immediate retraining for all shifts across its facilities. The refresher reinforced the importance of permit authorization for all maintenance and non-routine activities, strengthening awareness of roles, responsibilities, and communication protocols. The company’s blended operator training program—combining SOP reviews, hands-on simulation in a dedicated training center, and computer-based learning—ensures employees consistently demonstrate safe and reliable execution of critical tasks.     Following a review of non-routine maintenance risks, Optima Chemical developed and implemented a Non-Routine Hazard Assessment (NRHA) program to ensure safe execution of tasks without pre-established procedures. Since its launch, employees have completed more than 80 NRHAs, resulting in over 45 new written work instructions that improve consistency and prevent future incidents. The company’s three-week onboarding program integrates environmental, health, and safety training directly into operations instruction—reflecting a unified, safety-first approach to employee development.     Third Coast Pearland is recognized for advancing operational safety and community collaboration through proactive incident response, enhanced training, and strong emergency partnerships.
